1. The card is valid for 12 months from the date of issue
2. Birmingham residents are people that pay their Council
Tax to Birmingham City Council
3. The person to whom it was issued may only use the card.
It is not transferable and no more than one card will be issued
to any person
4. The card must be produced to obtain a discount otherwise
the full charge must be paid
5. The card can only be used at facilities listed in this Passport
to Leisure Card guide
6. To obtain Passport to Leisure rates for individual court
hire for badminton, squash and tennis, at least one of the players
must be a Passport to Leisure cardholder. Variable discount
apply at Billesley Indoor Tennis Centre and Community Leisure
Centres according to the number of Passport to Leisure cardholders
playing at the time
7. Discounts do not apply on multi-activity sessions, i.e.
Ladies Recreation and 50+ Recreation
8. To qualify for a discount on children's courses, i.e. Strikes
and Strokes, the child must be a valid Passport to Leisure cardholder
9. Last ticket for Erdington Leisure Centre is 3.30pm and
last ticket for Arthur Terry, Birmingham Sports Centre and Moseley
School Health & Fitness Centre is 5.30pm (Monday-Friday)
10. Cards may not be used after the expiry date
11. Cards must be renewed every 12 months. It is the responsibility
of the cardholder to renew their card before it expires
12. If a cardholder changes name, a new card may be issued
free of charge. Documentation will need to be provided as proof,
e.g. marriage certificate
13. An administration fee may be charged for replacement cards
14. Any changes of personal details or circumstances making
them ineligible for the card, must be notified to the Leisure
Card office
15. The giving of incorrect information with the intent of
defrauding the Council will result in appropriate action being
taken
16. We must protect the public funds we handle and so we may
use the information you provide in relation to this application
to prevent and detect fraud. This is in accordance with the
Data Protection Acts, 1984 and 1998
17. Birmingham City Council is committed to eliminating racial
harassment and attacks. The City Council reserves the right
to withdraw without compensation the Leisure Card of any person
deemed to be in breach of its Anti-harassment Policy
18. Birmingham City Council reserves the right to withdraw
the card and its advantages if the user abuses the scheme, other
customers or staff, or makes a false application. The card will
become invalid if tampered with in anyway
19. The loss or theft of a card must be reported immediately
20. Reduced rates for Passport to Leisure Card holders do not
exist for all services, e.g. club/group bookings and team sports
are excluded. In addition concessions are subject to availability
and may be altered or withdrawn without notice
21. The administration of this scheme is computerised and therefore
registered with the Data Protection Registrar in accordance
with the Data Protection Act 1984 and 1998
22. The card can not be used in conjunction with any other
promotions or special offers
