Proof of Eligibility for PTL


Leisure Card Details | Passport to Leisure Details

All applicants must produce evidence of their current address in addition to the relevant proof of eligibility when applying for and renewing a Passport To Leisure, select the most appropriate option from the box below:

People aged 60 years old and over
1.
Senior Citizens over 60 years Bus pass,
pension book,
driving licence,
passport or proof of date of birth
2.
Senior Citizens over 70 years Proof of date of birth
People receiving specified benefits or registered job seekers
3.
Job Seekers Allowance
(Contribution or Income based)
Letter from job centre confirming allowance
4.
Income Support Letter from Benefits Agency confirming allowance or allowance book
5.
Working Tax Credit Letter from Inland Revenue confirming allowance or allowance book
6.
Council Tax Benefit Council Tax Benefit confirmation letter or Council Tax Benefit book
7.
Housing Benefit Letter from Housing Benefits confirming benefits award
Full-time students over 16 years or trainees on Government Schemes
8.
Students Current 'full-time' Student Union Card, college enrolment letter or official letter from school sixth form confirming 'full-time' educational status
9.
Employment Training participants Letter of confirmation on headed paper from training agency or job centre, ES4 or income support book
People with a disability who receive benefit

Incapacity Benefit Letter from Benefits Agency confirming allowance or allowance book
11.
Disability Living Allowance
(personal or mobility help)
Letter from Benefits Agency confirming allowance or allowance book
Full-time carers who receive benefit

Invalid Care Allowance Letter from Benefits Agency confirming allowance or order book and name of carer
Children under 16 years whose parent(s) receive specified benefits

Child benefit book and evidence of parent(s) claiming one of 3, 4, 5, 6, 7 or 11
People with a disability who receive benefit and require assistance from a carer

Official letter from your doctor confirming the need for a carer. Applications should be made in the name of the person with a disability
Groups of people with special needs who require assistance

Applications must be made in writing to the manager of the leisure facility. The letter must be on official headed paper from group, organisation, school, club, society etc...
Foster Carers

Carer, Carer's children and/or looked after child. Letter of confirmation of carer status from Social Services

Asylum Seekers

Identity card or IS96, SAL1 or SAL2 forms

Please Note: All proofs of name and address must be less than 6 months old

  • proof of name and address - we will accept any of the following: driving license, council tax bill or book, gas/electric/telephone bill etc., bank statement, PO savings book, a family allowance book, medical card, a rental agreement or a letter certifying residence from an official body such as a local doctor, dentist, solicitor, bank manager or Housing Association manager
  • proof of age - only if applying for a senior citizen (over 70 years), student (over 16 years) or junior (under 16 years) card
Please Note: If your benefit is a 'joint' claim and your partner's surname is different, you must show evidence of sharing the same address, e.g. a letter addressed to both of you.
