Please Note: All proofs of name and
address must be less than 6 months old
- proof of name and address - we will accept any of the following:
driving license, council tax bill or book, gas/electric/telephone
bill etc., bank statement, PO savings book, a family allowance
book, medical card, a rental agreement or a letter certifying
residence from an official body such as a local doctor, dentist,
solicitor, bank manager or Housing Association manager
- proof of age - only if applying for a senior citizen (over
70 years), student (over 16 years) or junior (under 16 years)
card
Please Note: If your benefit is a 'joint' claim and your partner's
surname is different, you must show evidence of sharing the same
address, e.g. a letter addressed to both of you. |
