Renewing your Passport to Leisure | Corporate Leisure Card

How to apply for a Passport to Leisure Card
Applying for a Passport to Leisure Card could not be easier, just follow the steps below:

Step 1

Complete a Leisure Card application form either available at your local leisure centre or by calling 0121 464 5000.

You can apply online for a Passport to Leisure Card by clicking on one of the links below. Each one opens the same form, they can just be viewed in 3 different ways:

Step 2

Call in person to any Leisure Point site with:

- your completed signed application form OR the online application confirmation letter or reference number - young people under the age of 16 will need the signature of their parent or guardian

- payment

- proof of name and address - we will accept any of the following: driving licence, council tax bill or book, gas/electric/telephone bill etc., bank statement, Post Office savings book, a family allowance book, medical card or a rental agreement

Please note: If your benefits is a 'joint' claim and your partner's surname is different, you must show evidance of sharing the same address e.g. a letter addressed to both of you

- proof of age - only if applying for a senior citizen (60 years and over), student (16 years and over) or junior (under 16 years) card

- proof of eligibility - to speed up your application we have listed what you will need to show as proof of entitlement when applying for your card - click here proof of eligibility

PLEASE NOTE: If you have applied online you will still need to go to your local site to collect your card and take all eligibility criteria and payment with you. Applying online does not automatically mean you are eligable for a Passport to Leisure card.

Step 3
Smile - while you have your photograph taken at reception. Once your application has been processed you will be issued with your card and asked to sign it on the back. Your card will then be ready to use. Normally your card will be processed immediately although if the reception is really busy you may be asked to wait a while or to come back and collect your card at a later date.
How to renew your Passport to Leisure Card
Complete a Passport to Leisure Renewal Form available from any Leisure Point reception. Remember to bring proof of eligibility and payment. Until you reach 70 years of age as the card is then issued free of charge.
How to Pay
You may pay by cash, debit/credit card or cheque made payable to 'Leisure Point-Birmingham City Council'.
People aged 70 years and older
Residents*   FREE
Non-residents £4.00
All other categories
Residents*   £4.00
Non-residents £15.00

*Birmingham residents are people who pay their Concil Tax to Birmingham City Council

All cards are valid for 12 months. All cards show the expiry date on the back

Cardholders must remember to renew their card when it expires
